Blood pressure

Six years of “a little high.” Enough.

Your home readings in one trend, and the salt, sleep and movement that move the number, worked weekly against your own cuff. Your prescribers stay in charge of the medication. In Virginia, Clinical Care Navigation adds a nurse who helps you understand your results and gets your appointments scheduled and sequenced.

The reading nobody looks at becomes the reading your doctor acts on.

Today

  • You take readings at home. They live in a notebook or an app.
  • The cardiologist sees one reading, in the office, after you rushed to get there.
  • Two doctors adjust the dose. Neither knows.
  • The new pill makes you dizzy. You leave a voicemail.

With Simbioza

  • Your home readings logged in your dashboard, ready before every appointment
  • Sodium, sleep, and movement tracked against your mornings, worked weekly with your coach
  • Questions about your two prescriptions written down, for your doctors to sort out
  • Questions for the appointment prepared in your dashboard
A blood pressure reading at the kitchen counter

Before coffee. Same chair. Every morning. That’s the reading that counts, and the one your cardiologist has never seen. You log it in your dashboard, and it's ready before every appointment, with the questions to ask.

Thirteen weeks of mornings.

Weekly average of your home readings. The red ones are the weeks your doctor had never seen before.

Numbers illustrative.

Four levers your doctor doesn’t have time for.

Each one moves the number. Your coach works them weekly, against your own readings.

Salt

Where it hides

Restaurant nights show up in your mornings. Two swaps, tracked.

Sleep

Under six hours

Short nights, high mornings. A wind-down plan that’s in writing.

Move

Evenings, earlier

A walk before dinner beats one after. Logged in your dashboard.

Pills

The way they’re prescribed

Side effects logged in your dashboard and ready for your prescriber, so you don’t quietly stop.

Lifestyle first

Sodium, movement, sleep and weight are the first things clinical guidelines ask people with high blood pressure to work on. Coaching is how changes like that actually stick. Results vary.

Questions people with high blood pressure ask first.

Can you get me off blood-pressure medication?

Only your prescriber can change your medications. What we can do is get your readings and lifestyle changes in front of them with the data to support the conversation.

Do I need a home cuff?

Yes, and most people already own one. You log your morning readings in your dashboard, so they're ready to bring to your doctor.

Two doctors manage my blood pressure. Is that a problem?

Only when they don't know what each other prescribed. Writing down what each doctor prescribed is the first thing we do together, and the questions for both doctors go on your list; sorting the prescriptions is theirs to do.

Do you talk to my doctors or handle referrals?

Your coach works on nutrition, exercise, sleep and stress; clinical questions go to your own doctor. We don't file appeals or interfere with a specialist's decisions. In Virginia, Clinical Care Navigation adds a nurse who helps you understand your results and benefits and gets your appointments scheduled and sequenced.
