Three prescribers. One list.
Most diabetes cases have a PCP, an endocrinologist, and a cardiologist, each adjusting something. Nobody holds the whole list. Your file does, and the questions for all three go on it.
Without a plan
- Metformin dose changed by the PCP; the endocrinologist doesn't know
- A CGM letter from the insurer nobody explains
- Referral placed in January, appointment in May
- Labs drawn after the appointment, not before
With one
- One medication list, kept in your file, with the questions for each doctor
- The letter read with you and the next step written down
- The appointment booked, and prepared for
- Results in your file before you walk in, with what each one measures explained

Your meter knows more than your appointments do.Which breakfast, which walk, which late snack: that’s what your coach works on with you, week by week. What your readings mean for your treatment is your doctor’s call.
Ninety days, by the numbers.
Baseline in your own words: food, movement, sleep, stress. The first change to test, chosen together.
First food swap tested against your readings. Questions for your endocrinologist prepared.
Strength twice a week, protein at every meal, walking after the biggest meal. Kept what worked.
Ninety-day review: how you feel and function, in your own words. Continue, change focus, or graduate.
Numbers illustrative.
Tested one at a time.
Not a diet. Your coach changes one thing, you try it for two weeks, and together you keep it or drop it. That’s the weekly session.
Structured lifestyle coaching is the most-studied approach for prediabetes, in the NIH's Diabetes Prevention Program. Results vary; we describe the work and the cadence, not a number.

Questions people with diabetes ask first.
Can you help me get off metformin?
Only your prescriber can change your medications. Your coach helps you build the habits and prepares the questions, so that conversation with your doctor is a good one.
My doctor said "prediabetes." Is that serious?
That's a question for your doctor; the ranges are theirs to interpret. What we can say is that prediabetes is where lifestyle coaching has the most evidence behind it.
Do you talk to my doctors or handle referrals?
Your coach works on nutrition, exercise, sleep and stress; clinical questions go to your own doctor. We don't file appeals or interfere with a specialist's decisions. In Virginia, Clinical Care Navigation adds a nurse who helps you understand your results and benefits and gets your appointments scheduled and sequenced.
